1. I understand that PrepMe Summer School is a free program with limited spaces, and submission of this form does not guarantee a spot, but enters my child into consideration.

2. I grant permission for my child to participate in the PrepMe Summer School.

3. I understand that this is an online program and I am responsible for providing the necessary internet connection and devices for my child to participate.

4. I give my consent for my child to have their camera and microphone turned on during online sessions to support interactive participation.”

5. I authorize PrepMe to contact me via email and phone regarding my child's application and participation.
